Home instruction is the provision of one-to-one or small group instruction in the student’s place of residence or other appropriate setting due to a health condition, need for treatment, court order or exclusion from general education for disciplinary or safety reasons.
Home Instruction must be requested by the parent or legal guardian and be accompanied by a physician's note stating the reason and length of time the student is expected to be on home instruction.
The length of time a student can be out on home instruction is 60 calendar days. If the students requires home instruction beyond 60 calendar days, a request for renewal may be made by the parent or legal guardian and submitted with a new note from the student's physician. All requests for home instruction extending beyond 60 calendar days will result in the school district's physician referring the student to the Child Study Team for an evaluation.
Forms to request home instruction are attached for your convenience. Please submit forms to the Department of Student Services.
