Steps for Students Approaching Graduation

  • Perform Care/Children’s System of Care

    **If you have an emergency situation involving your child that is life-threatening, seek help immediately by dialing 911.

  • DVRS (Department of Vocational and Rehabilitation Services) – Pre-Employment Services (Pre-Ets) 
    Services you may need to get and keep a job, vocational counseling and guidance. 
    How to apply: Submit below forms to your current case manager or Transition coordinator 

    1. Pre-Employment Transition Services Student Referral Form
    2. Release of information form for DVRS

    NJCBVI • (Commission for the blind and visually impaired) To request Pre-ETS for a student who is blind, deaf-blind, or visually impaired, please contact the student’s NJCBVI teacher of the blind or visually impaired for a referral (if applicable) or contact a regional NJCBVI office listed here: https://www.state.nj.us/humanservices/cbvi/facilities/

  • There are three types of guardianship:

    1. “Guardianship of the person and estate. The guardian is responsible for the individual’s healthcare and financial management. This applies when the ward also has assets that qualify as an estate.
    2. Guardianship of the person only. This applies when the ward needs to be looked after but does not have assets to manage.
    3. Guardianship of the estate only. The guardian only manages the ward’s financial affairs.”

    Apply for SSI or Supplemental Security Income: Here is the link to set up the initial SSI meeting. Go to Disability | SSA or Call 1-800 772-1213 8am to 7pm Monday-Friday to make an appointment at your local office

  • DVRS Department of Vocational and Rehabilitation Services Vocational Rehabilitation Agencies assist individuals with disabilities to prepare for, obtain and/or keep suitable jobs.
